Providing a range of preventative Community Health and Home Based Aged Care Services to the District Council areas of Elliston, Streaky Bay and Le Hunte.

Aim:

To provide wherever practical a range of Service options to all clients that will enable them to remain within the community and to return to near as normal lifestyle as it is possible for them.

Ensure that services cater for those convalescing from an episode of ill health, as well as the frail aged or younger disabled and their carers experiencing difficulties with daily living tasks or those tasks that threaten their ability to remain at home within the community.
